Aledade was founded in 2014 by Farzad Mostashari, who previously served as the National Coordinator for Health Information Technology under the Obama administration, along with co-founders Travis Broome and Edwin Miller, based on the observation that independent primary care physicians, who make up a large share of U.S. primary care but often lack the scale and data infrastructure of large hospital systems, were being left behind as U.S. healthcare shifted from traditional fee-for-service billing toward value-based care arrangements that reward providers for improving patient outcomes and reducing unnecessary costs rather than simply billing per visit or procedure. Aledade's model is to partner with independent and small-group primary care practices, organize them into accountable care organizations that can participate in value-based contracts with Medicare, Medicaid, and commercial insurers, and provide the data analytics, workflow software, and administrative support needed to actually succeed under those contracts, such as identifying which patients are at highest risk of a costly hospital admission and need proactive outreach. In exchange, Aledade typically shares in the savings and quality bonuses that its partner practices earn under these value-based arrangements, aligning its own revenue with helping practices improve patient outcomes and reduce unnecessary utilization rather than charging a flat software licensing fee. The company has grown to work with thousands of independent practices across dozens of U.S. states, a large network of its kind, and has raised substantial venture funding from investors including General Catalyst, Meritech Capital, and Venrock, reaching unicorn status as value-based primary care has attracted growing investor and policy attention as an alternative to traditional fee-for-service medicine.
